How It Works

We follow a structured and person-centred onboarding process to ensure the safe and effective delivery of care services. Below is an outline of our standard procedure when we receive a referral from an Integrated Care Board (ICB) or local authority.

Step 1

Referral and Capacity Confirmation Upon receipt of a referral,

we promptly assess our capacity and respond to confirm whether we are able to take on the care package.

Step 2

Initial Contact and Assessment Planning

Once the support plan is shared with us, we seek permission to make contact with the service user or their next of kin. After establishing communication, we arrange a suitable date and time for an initial assessment.

Step 3

Costing and Contractual Agreement

While the risk assessment and care plan are being finalised, our Accounts Team prepares a cost breakdown for the proposed care package.

Step 4

Package Commencement

A mutually agreed start date and time is arranged in collaboration with the service user or their next of kin.

Step 5

Staff Induction and Competency.

For carer-led packages, all carers are shadowed by a qualified staff nurse until they demonstrate competency and confidence in delivering the care, after which they are formally signed off by the Clinical Lead. All carers receive mandatory training, including specialised training in complex care. For nursing packages, we typically allocate highly skilled, specialist-trained nurses, including those with backgrounds in ITU and A&E.

Digital Care Management and Communication

We use a secure care management software, to maintain all documentation and enhance communication
